Hospital to Home (H2H) is a digital solution that removes the complexity in the discharge to assess process. A solution for Adult Social Care and Health partners, H2H provides a single source of truth, real time data and comprehensive reporting .

The UKRI Healthy Ageing Challenge funding will see the extension of H2H into Intermediate Care, providing a new module that manages the four IC pathways. A second project will resolve the Interoperability challenge for our customers, integrating H2H with key Health and Adult Social Care Systems. We can ensure integration to FHIR standards. 
These two projects will give Adult Social Care teams and their partners digital solutions that enable them to provide the best standards of care particularly to our ageing population.
